The Foundation for Black Women’s Wellness (FFBWW) is seeking passionate and committed Community Health Workers to help advance our commitment to Empowering a Generation of Well Black Women. These (three) positions will play a key role in realizing our organization’s mission to energize, mobilize and support Black Women to transform their lives through education, advocacy, and powerful partnerships. 

FFBWW Community Health Workers (CHWs) serve as advocates and liaisons for Black women and families in securing vital health and community resources and achieving optimal health and wellness in community and healthcare settings. They build trust and promote engagement in the Foundation’s health promotion and prevention initiatives, while assisting community members to navigate and access healthcare and other family-supporting resources to meet their health and wellness needs. Ideal candidates are unwavering advocates of Black women’s health who are committed to building and sustaining relationships and ensuring that Black women have access to the services and resources needed to sustain their health and the health of their family. Desired candidates are solutions-oriented collaborators and problem solvers who can juggle multiple, often competing priorities, and deliver consistently excellent service to the women they serve, and to community partners. These energetic, highly-organized professionals will play a key role in ensuring the continued success of the Foundation’s community health initiatives. 

Specifically, Community Health Workers will be responsible to:

Access, Navigation & Advocacy 

  • Connect women and birthing persons to the Foundation’s health promotion and prevention education and initiatives and other relevant community resources that address critical health needs.
  • Support women and birthing persons in identifying issues/barriers that affect their overall health and develop plans and goals to improve their wellbeing. 
  • Link participants to health care/social service resources; aid participants in identifying and completing applications for services when appropriate.
  • Serve as a liaison for participants, coordinating referrals between multiple services, assisting with care initiation and facilitating communication between participants and care/service providers. 
  • Provide support and advocacy during care, resource, and medical visits to ensure that participants’ overall well-being is prioritized. 
  • Deliver health information using culturally appropriate approaches.
  • Develop and maintain effective working relationships with participants and community partners. 

Case Management & Informal Counseling

  • Provide informal counseling, support and follow-up to participants.
  • Document all encounters and contacts made with participants or on participants’ behalf; complete and submit monthly reports, and maintain comprehensive files with a high degree of accuracy
  • Regularly review participant progress in reaching health, wellness, and related goals and collaborate to find creative solutions. 
  • Monitor quality of services provided and suggest updates to internal processes accordingly.
  • Attend regular team meetings to facilitate case planning and collective problem-solving that supports participant health and overall program/service effectiveness.
  • Maintain participant confidentiality at all times.
  • Other duties as assigned.


  • Associates degree in health related area required. Bachelor’s degree preferred. Prior or current Community Health Worker or related experience and CHW training and/or certification are highly desired.
  • A passionate advocate for Black Women’s health, and commitment to bringing women and partners together to improve the health of Black Women, families, and communities;
  • Strong knowledge of community, neighborhoods, resources, and dynamics impacting Black women/family/community health.
  • Previous working experience in community outreach, case management, health education, or a related field;
  • A hands-on learner who thrives in a collaborative, fast-paced environment with frequent changing priorities
  • Exceptional organizational, analytical, and problem-solving skills—must be able to think on your feet and outside the box;
  • A strong team player and self-starter with a desire to take on new challenges; 
  • Excellent time management skills; must be flexible and able to work outside of normal business hours as needed;
  • Excellent organizational abilities, impeccable communication, and people skills.
  • Firm grasp of MS Office and computer technical skills for word processing and participant information management.
  • Ability to travel locally up to 75% of the time in the community.

Job Details

This is a full-time benefits-eligible position with a minimum starting salary of $42,500 per year. This is a grant funded position for two years. Continuation of employment subject to funding availability.
