FFBWW Community Health Worker (CHW) serves as a frontline health worker dedicated to empowering Black women and families in securing vital social determinants of health (SDoH) supports and connecting Black women and their families to critical community-based resources, to achieve optimal health and wellness in community and healthcare settings. Builds trust and promotes engagement in the Foundation’s health promotion and prevention initiatives, while assisting community members to navigate and access healthcare and community-based SDoH resources to meet their health and wellness needs. The ideal candidate is an unwavering advocate of Black women’s health who is committed to building and sustaining relationships and ensuring that Black women have access to the services and resources needed to sustain their health and the health of their family. The desired candidate has experience with community engagement and outreach, has experience with case management and documentation, is a solutions-oriented collaborator and problem solver who can juggle multiple, often competing priorities, and deliver consistently excellent service to the women they serve, and to community partners. This energetic, highly-organized professional will play a key role in ensuring the continued success of the Foundation’s community health initiatives.

The Foundation for Black Women’s Wellness (FFBWW) is seeking a passionate and  committed Community Health Worker to help advance our commitment to Empowering a  Generation of Well Black Women. This position will play a key role in realizing our  organization’s mission to energize, mobilize and support Black Women to transform their lives  through education, advocacy, and powerful partnerships.

Duties/Responsibilities:

Access, Navigation & Advocacy  

  • Community Outreach and Engagement: Proactively engage with members of the community to assess their Social Risk Factors, address their social needs, build rapport, and establish trusting relationships. Attend community events, and utilize various communication channels to disseminate SDoH & Health information and promote health literacy.
  • Resource Navigation: Assist clients in accessing community resources and support services that address their specific social needs. This could involve providing information about food banks, housing assistance programs, job training opportunities, childcare services, financial assistance programs, and educational resources.
  • Connect women and birthing persons to the Foundation’s health promotion and prevention education and initiatives and other relevant community resources that address critical health needs.
  • Assessment of SDoH: Conduct comprehensive assessments to identify the social determinants affecting the health and well-being of clients. This may include factors such as housing stability, food security, access to transportation, employment status, education level, and social support networks.
  • Link participants to health care/social service resources; aid participants in identifying  and completing applications for services when appropriate.
  • Serve as a liaison for participants, coordinating referrals between multiple services,  assisting with care initiation and facilitating communication between participants and  care/service providers.  
  • Provide support and advocacy during care, resource, and medical visits to ensure that  participants’ overall well-being is prioritized.  
  • Deliver health information using culturally appropriate approaches.
  • Develop and maintain effective working relationships with participants and community  partners.

Case Management & Informal Counseling

  • Assessment of SDoH: Conduct comprehensive assessments to identify the social determinants affecting the health and well-being of clients. This may include factors such as housing stability, food security, access to transportation, employment status, education level, and social support networks.
  • Case Management: Provide ongoing support and follow-up to ensure clients' needs are met and their goals are achieved. Coordinate with other members of the healthcare team, social service agencies, and community organizations to develop and implement holistic care plans that address clients' social, emotional, and physical needs.
  • Crisis Intervention: Respond to crises or emergencies related to social determinants, such as homelessness, food insecurity, domestic violence, or mental health crises. Provide immediate assistance, connect clients with emergency resources, and facilitate access to appropriate support services.
  • Provide informal counseling, support and follow-up to participants.
  • Document all encounters and contacts made with participants or on participants’  behalf; complete and submit monthly reports, and maintain comprehensive files with a  high degree of accuracy
  • Regularly review participant progress in reaching health, wellness, and related goals  and collaborate to find creative solutions.  
  • Monitor quality of services provided and suggest updates to internal processes  accordingly.
  • Attend regular team meetings to facilitate case planning and collective problem solving that supports participant health and overall program/service effectiveness.
  • Maintain participant confidentiality at all times.
  • Other duties as assigned.

Required Skills/Abilities:

  • A strong team player who is also a self starter with a passion for improving birth outcomes for families.
  • Tech Savvy with a firm grasp of MS Office, Google Business Suite, and various business and project management software.
  • Strong knowledge of community, neighborhoods, resources, and dynamics impacting Black women/family/community health.
  • Previous working experience in community outreach, case management, health education, or a related field.
  • A hands-on learner who thrives in a collaborative, fast-paced environment with frequent changing priorities
  • Exceptional organizational, analytical, and problem-solving skills—must be able to think on your feet and outside the box.
  • Excellent time management skills; must be flexible and able to work outside of normal business hours as needed.
  • Excellent organizational and leadership abilities, impeccable communication and people skills.
  • Ability to travel locally up to 75% of the time in the community.

Education and Experience:

  • High school diploma required.
  • Ability to work independently and as part of a team.
  • A passionate advocate for Black Women’s health, and commitment to bringing women and partners together to improve the health of Black Women, families, and communities.

Compensation & Benefits:

  • Salary is commensurate with experience.
  • FFBWW offers a competitive and comprehensive benefits package including employer paid health care benefits, dental coverage, vision coverage, health reimbursement arrangement (HRA), 401k plan with employer match, optional life insurance and disability coverage.
  • FFBWW offers 160 hours of  Paid Time Off (PTO) in a calendar year (hours are prorated for new hire employees). FFBWW also provides 12 paid holidays per year, plus a bonus day off during your birth month for your birthday.

Work Location:

  • This position will be based out of our administration headquarters in Madison, WI 53719,  with flexibility for a hybrid work schedule upon approval from the supervisor.

Physical Requirements:

  • Prolonged periods sitting at a desk and working on a computer.
  • Must be able to lift up to 15 pounds at times.
